This ANSI Z535.5 Danger Do Not Operate Equipment Locked Out photo lockout tag is an ideal tagout solution for any facility’s safety program. Unlike standard lockout tags, lockout tags with photos allow personnel to place a small image of their face on the photo area and use the self-laminating flap to seal the image to the tag. This also protects any writing on the tag. Lockout tags with photos makes the lockout procedure more personal by letting others know that this person’s life could be in danger if the lockout is disregarded.
The photo box measures 1" x 1" but the area around the photo box measures 2-1/4" wide x 1" tall. There is also space for the worker to write their name, date, and leave any comments before they laminate the tag. Durable UV resistant black nylon ties that have 50 lb. of tensile strength are included for easy attachment.
