How To Apply Self-Laminating Flap:
1. Pull back the laminate from the bottom of the tag.
2. Peel away the liner from the laminate.
3. Place your photo on the tag.
4. Press the laminate on top of the tag, starting from the header and moving down.
This ANSI Z535.5 Danger Equipment Locked Out My Life Is On The Line photo lockout tag is an ideal safety solution for any lockout tagout program. Unlike standard lockout tags, this tag is specifically designed to allow a worker to place a photo of their face on the tag. The photo is then secured using the self-laminating flap; which also protects any writing. This makes the lockout procedure more personal by letting others know that this person’s life could be in danger if the lockout is disregarded.
The photo box measures 1" x 1" but the area around the photo box measures 2-1/4" wide x 1-3/4" tall. The back of the tag remains free of lamination to allow for writing at any time. Durable UV resistant black nylon ties that have 50 lb. of tensile strength are included for easy attachment.
